This form can be used by those
individuals who do not have access to online services. You may request sign
language interpreting services via fax by printing this form and making copies
for your future fax requests. You may then fax in your interpreting service
requests whenever needed.
If the date you are requesting
an interpreter is LESS THAN 48 HOURS (2 BUSINESS DAYS) from the date you are
sending the fax request you must CALL OUR OFFICE, (in addition to sending the
fax request).You may use our 24 HOUR TELEPHONE 800 NUMBERS to advise us of your
request. If your request is in the future you can just fax in your request.
